The form will point out any restrictions. If you do not want to order online, you can still fill out this form, print it, and fill out the credit information by hand.

Personalization:
Double check spelling and dates. We stamp what you write!
Form accepts capital letters only. Established will appear as "est" on the pot.
Use asterisks (*) for stars in personalization boxes.
If you do not type "THE" it will not be stamped. "THE" is on its own line, do not count it as part of the 14 character limit.
Apostrophes are not used unless requested.
