Now that you have selected a shape, fill in the personalization boxes. Although we show specific decorations with each shape, you can personalize your selection any way space permits. The form will point out any restrictions. If you do not want to order online, you can still fill out this form, print it, and fill out the credit information by hand.

Personalization:
Double check spelling. We stamp what you write!
Form accepts capital letters only. Established will appear as "est" on the pot.
Use asterisks (*) for stars in personalization boxes.
If you do not type "THE" it will not be stamped. "THE is on its own line, do not count it as part of the 14 character limit.
Current year used unless otherwise requested under "Special Instructions" at checkout.
Apostrophes are not used unless requested.
